Under Restraint: No Summary of Incident Summary of How the Death Occurred: (max. 30,000 characters) Summary: On Saturday, June 19, 2021 at approximately 1626 hours, a Code 1 was initiated in unit LA due to inmate McClain, Willie #223682 having a medical episode. Medical staff arrived and escorted inmate McClain to medical for further evaluation via wheelchair. Inmate McClain was being evaluated by medical staff. Per medical staff, inmate McClain would be sent to University Hospital via ambulance, however, inmate McClain soon became unresponsive. At approximately 1710 hours, a Code 1 Blue was initiated. Medical staff performed CPR until San Antonio Fire Department arrived. SAFD arrived and continued their life saving measures. Inmate McClain was pronounced deceased at 1739 hours.
