Under Restraint: No Summary of Incident Summary of How the Death Occurred: (max. 30,000 characters) Summary: On 10-16-19 Temple Police Department Officer(s) arrested Mr. Richard Allen Butler w/m dob 10/3/58 (61 years old) for Assault w/ Bodily Injury to Family Member. Richard Butler was brought to the Bell County Jail, he was on Oxygen, (carried in a harness), and had a walking cane. Bell County contracts with Wellpath (a private medical provider) to provide medical services to Bell County Jail Inmates. Inmate Butler was screened Custodial Death Report :: Page 6 of 7 --- PAGE BREAK --- by a Nurse before being accepted into the Bell County Jail. Inmate Butler reported to the Nurse he suffered multiple illnesses but denied suffering any pain or discomfort at the time of his assessment. Inmate Butler was booked into the Bell County Jail at 9:23pm. Due to numerous medical issues Inmate Butler listed on his Intake Screening Form, he was never placed in a holding cell. Instead, he was place an open area in the Intake area next to video visitation monitors. He was located in directly in front of the Booking Desk and Nurses Station where he could be observed by Jail and Nursing Staff. Inmate Butler remained at this general location for his stay at the Jail. Inmate Butler was attended to by the Nurse during the night for nausea, spitting up and headache. Inmate Butler's blood sugar was checked in the early morning hours and he was given treatment by the Nurse. Inmate Butler was assisted by jail staff in and out of his chair to change his uniform. On the Morning of 10/17/19, Inmate Butler, who was sleeping at the time, blood sugar was checked again by the Nurse on duty. Inmate Butler was given another treatment by that Nurse. Approximately one hour later, a Jail Corporal requested the Nurse to assist Inmate Butler, who's eyes were open but he was unresponsive. The Nurse called for the Wellpath Doctor that was on duty to assist her. The Wellpath Doctor arrived and after assessing Richard Butler, she called for Inmate Butler to be transported to Baylor Scott and White Hospital Emergency room via EMS. Inmate Butler was later admitted to Baylor Scott and White Hospital. On 10/18/19 at 12:30pm the Bell County Jail was notified Richard Butlers' life support was turned off at the request of his family. Richard Butler was pronounced dead at Baylor Scott and White Hospital at 12:21pm by Justice of the Peace Cliff Coleman. This incident is being investigated by DPS Texas Ranger Adam Russell.
