Summary of Incident
Summary of How the Death Occurred: (max. 30,000 characters)
Summary:
On May 19, 2017, decedent was discovered unresponsive by a juvenile correctional officer conducting routine
room checks on 15 minute intervals. The decedent had utilized an agency issued bed sheet to hang himself
from a concrete partition in his assigned room at the Ron Jackson State Juvenile Correctional Complex in
Brownwood, TX. Though decedent had been diagnosed with a mental condition, including a prior suicide
attempt at a county facility, there was no evidence of recent statements or suicidal activity observed or noted
by T.J.J.D. staff. Room checks and video surveillance images do not show anyone else entering or leaving
the decedent's room immediately preceding his suicide attempt. Upon discovery, staff immediately attempted
life saving measures, and local emergency services were notified. The decedent was successfully
resuscitated by EMS personnel on scene before being transported by ambulance to the Brownwood Regional
Medical Center. The decedent was later transferred to Cooks Children's Medical Center in Fort Worth, TX for
additional care. Testing at that facility determined that decedent had suffered severe brain damage caused by
hypoxia, and the biological father requested he be transferred into hospice care in Odessa, TX. The court of
jurisdiction was notified of the transfer. The decedent passed at this father's residence in Ector County on
May 31, 2017. The Ector County Medical Examiner's Office ordered the autopsy which was conducted by the
Tarrant County Medical Examiner's Office in Fort Worth on June 2, 2017. Dr. Peerwani indicated that the
preliminary cause of death was suicide, and the manner of death was by strangulation.
