Under Restraint: No Summary of Incident Summary of How the Death Occurred: (max. 30,000 characters) Summary: On January 23,2022 Inmate Clements, Jerry fell in his cell at the Childress County Jail. Inmate Clements was transported to Childress Regional Medical Center. At 09:03am Childress County Sheriff Office contacted Hall County Sheriff Office that Inmate Clements was at the ER. Deputy V. Marasco was dispatched to Childress Regional Medical Center for security of Inmate Clements. Deputy V. Marasco left the Hall County Jail at 09:36am and arrived at 10:03am. At 13:52pm Inmate Clements was admitted to Childress Regional Medical Center, he had tested positive for COVID 19 and pervious heart condition . He was assigned to room 313. On January 25,2022 at approx 06:30am, Inmate Clements stated to Deputy K Dwight that he need to go to the restroom and that he had called for the nurse. A nurse did not come to assist Clements. Deputy K Dwight ask Clements if he could walk with his walker. He said yes, Deputy K Dwight assisted him out of the bed and gave him his walker. Inmate Clements walked about four feet, when he stated that he was very dizzy. At that time Deputy K Dwight noticed that Clements was wobbling, she got behind Clements and assisted him to the floor. Deputy K Dwight helped Clements into a sitting position, and called for a nurse to come to the room and assist Clements back into the bed. At 06:45 am a nurse had not came to the room, Deputy K Dwight stepped into the hall way and told a nurse of the situation. Three nurses then came to the room and assisted Clements to the bed. When Deputy K Dwight arrived for shift at 19:00pm, she noticed that Clements was disoriented, making statements of being in Levelland, Texas. And talking about his wife coming to see him.
