Summary of How the Death Occurred: (max. 30,000 characters)
Summary:
On Sunday, September 20, 2026, at approximately 6:53 a.m., Lt. Felipe Olivares contacted me and notified
me of a death in custody. Lt. Olivares explained that an inmate from the Carrizales Detention Center had
suffered a cardiac arrest while being transported to the hospital.
Following protocol, I immediately contacted the Texas Rangers and was advised that Ranger Rodney
Henderson would be responding. I then proceeded to the Carrizales Detention Center, located at 7100 Old
Alice Road in Olmito, Texas.
Upon arrival, I proceeded to the infirmary, where I obtained the following information:
Inmate Information:
Name: Javier Sepulveda
D.O.B.: 11/01/1967
Age: 58
Housing Location: Charlie 5 POD

Methotrexate Sodium 50 mg
Prednisone 10 mg
I then made contact with Jailer Angel Ruedas, D.O.B. 06/12/2007, who explained that he was assigned as the
runner for his shift and responded to a medical episode in Charlie 5. Jailer Ruedas stated that when he
arrived, inmate Sepulveda was on the floor between two bunks experiencing seizure activity. Sepulveda
appeared to be having difficulty breathing and was opening and closing his eyes.
Further investigation revealed that medical staff was notified and responded shortly thereafter. Ranger
Rodney Henderson subsequently arrived at the Carrizales Detention Center, processed the scene, and
assumed responsibility for the investigation. Justice of the Peace Cindy Hinojosa requested that an autopsy
be conducted.
No further action was taken.
