Summary of Incident
Summary of How the Death Occurred: (max. 30,000 characters)
Summary:
Decedent was only person in jail cell at the time of the incident. Decedent was able to tear a jail mattress
fabric apart in to thin strips to create a knot which he put around his neck. Decedent tied one end of the fabric
strip to the jail cell door and placed the knot end around his neck. Decedent unsuccessfully attempted suicide
twice with the thin mattress fabric strips, but due to his weight the fabric strips broke. Decedent then reached
in to jail toilet with hands and spread water on the floor on the jail cell. On Decedent's third suicide attempt he
created and used a much thicker mattress fabric strip and tied it to the jail cell door and then placed the open
knot end around his neck. The mattress fabric strip/knot that he put around his neck on the third attempt did
not break which resulted in the suicide death of the decedent. The incident was captured on jail surveillance
cameras. The Decedent's death was caused by self-directed injurious behavior with an intent to die as a
result of the behavior. No other person entered or exited the jail cell during the incident.
