Custodial Death Report :: Page 6 of 7
--- PAGE BREAK ---
Officers responded to a residence after receiving a report of two individuals fighting. Upon arrival, they were
allowed inside by a resident and found Domonick Hernandez being restrained by family members following an
alleged assault inside the home. The residence showed signs of a struggle, including damaged furniture and
broken glass.
Officers observed Hernandez exhibiting erratic behavior, including disorganized speech, apparent
hallucinations, and signs consistent with a possible mental health or substance-related crisis. Due to concerns
for his condition, emergency medical services and the fire department were requested to respond.
While officers attempted to speak with Hernandez, he continued to behave erratically and attempted to get up
despite repeated verbal commands to remain on the floor. Family members advised officers that Hernandez
had allegedly assaulted family members and had used or displayed a knife during the incident. For the safety
of everyone present, officers placed Hernandez in handcuffs after he actively resisted being detained. During
the detention, officers repositioned him onto his side after he stated he was having difficulty breathing.
While officers began securing the scene and speaking with witnesses, one officer located a knife believed to
have been used during the incident. At that time, officers discovered Hernandez was unresponsive and not
breathing. They immediately requested emergency medical personnel to expedite their response, removed
the handcuffs, and began assessing his condition.
Emergency medical personnel and firefighters arrived shortly afterward and initiated lifesaving measures,
including CPR.
Domonick Hernandez was transported by EMS to the hospital where he was pronounced deceased after life
saving procedures were unsuccessful.
