Summary of Incident
Summary of How the Death Occurred: (max. 30,000 characters)
Summary:
On 06/30/2019 at approximately 1438 hours, Officers were notified of a traffic violator traveling south bound
on 3000 N I-35E, Lewisville, Denton County, TX, 75067. The complainant traveling in the same direction on
I-35 identified a black Chevy pickup with garden tools in the bed of the pickup that was going approximately
100 mph while switching lanes on I-35E. The complainant stated that the pickup almost hit the barrier wall
while on the interstate.
At 1438 hours, Officer STEINLE entered the 1300 block of I-35 SB and located a black Chevy pickup with
garden tools in the bed of the truck traveling on the inside lane of I-35 SB, going approximately 100 mph on
the interstate. Officer Steinle drove behind the black Chevy pickup (TX-LP CR55701) and turned on his
emergency lights for a traffic stop. The vehicle did not stop and continued going the same speed southbound
on I-35. The vehicle then exited at the 121 state highway flyover from I-35. Officer Steinle continued to follow
the vehicle onto 121 State highway southbound. Officer Steinle did not call a pursuit at the time due to
thinking that the vehicle was trying to find a safe place to stop. The vehicle then exited at the 500 block of 121
SH and on to the south bound frontage road. The vehicle continued to travel southbound on frontage road
towards MacArthur Blvd. The vehicle then turned right onto MacArthur Blvd and continued traveling
northbound in the far right lane. The vehicle passed several businesses with parking lots on MacArthur Blvd.
Officer Steinle decided to turn on his siren while traveling behind the vehicle on MacArthur Blvd.

At approximately 1445 hours, as the vehicle was in the middle of intersection of Highland Dr and MacArthur,
Officer Steinle observed the driver with his right hand put a gun to the right side of his head and immediately
saw the driver shoot himself in the head shattering the glass of the driver’s window. Officer Steinle observed
the vehicle stop for a brief second and then continued to move northbound passing the highland Dr
Intersection. Officer Steinle then pulled in front of the moving vehicle while matching its speed and slowed the
vehicle down by placing the rear bumper of the squad card onto the front bumper of Chevy pickup. The black
pickup came to complete stop at 2700 MacArthur Blvd. Officer Steinle, exited his squad car and held the
vehicle at gunpoint behind the left rear squad door until additional officers arrived at the scene.
Additional officers arrived at the scene and approached the vehicle. Due to the vehicle still being in drive,
Officers shattered the driver windows glass fully with a baton and unlocked the doors of the vehicle. Officers
observed a white male with a gunshot wound to head in the driver’s seat. Officers observed a silver Smith
Wesson 40 caliber handgun (FXM2210) in the floorboard of the driver’s seat. Officers cleared the rest of the
vehicle and observed blood splatted over the driver’s seat and center console. Officers then pulled the white
male from the vehicle and carried him off the street and onto the sidewalk of MacArthur. As officers were
beginning to apply medical aid, a citizen identified as PATRICK SKINNER approached the scene and wanted
to help. SKINNER stated he was a combat medic that served on multiple deployments. SKINNER helped
officers apply wound care to the head of the white male. Officers observed an entry wound in the right temple
area of the head and an exit wound above the left ear. Officers observed an odor of alcohol coming from the
male. Officers noticed that the male also had grass on his legs and shoes as if he was working outside.
Officers retrieved a wallet in the vehicle that included a Texas DL and were able to identify the white male as
DEREK COLLINS. Lewisville Engine 5 and medic 3 arrived on scene and immediately started aid on
COLLINS. COLLINS was unconscious but still breathing at the scene.
At 1512 hours, MEDIC 3 with Officer O'Hare transported COLLINS to Baylor Scott & White Medical Center-
Grapevine. Officer Wright left the scene and followed behind enroute to Baylor Scott & White Medical Center-
Grapevine. Officers arrived at the medical center at approximately 1527 hours. Detective Short also arrived at
the hospital to further investigate. Medical staff continued to work on COLLINS and were able to control the
bleeding and maintained a pulse. Officers gathered the clothing of COLLINS for evidence. Medical staff also
placed GSR bags on the hands of Collins for gun residue. While receiving care at the hospital, Officers
observed COLLINS to have several tattoos on his chest and throughout his right arm. Medical staff informed
Officers that Collins blood alcohol limit was not above the legal limit. Officers took pictures of COLLINS while
in the hospital bed. Officers were notified by hospital staff that they were waiting to see if COLLINS would be
able to donate his organs.
Dispatch conducted a NCIC computer check of COLLINS and found him to have no active warrants. Dispatch
also found the Smith and Wesson 40 caliber handgun to come back clear. A criminal history check of
COLLINS showed him to have two prior evading convictions out of Collin County. COLLINS was put on
probation on 03/14/2018. The tattoos listed in the criminal history of COLLINS matched with the tattoos
observed on his body.
The address listed on COLLINS TEXAS DL was 300 Appalachian Way, McKinney, TX. McKinney PD was
notified to check the address for a next of kin notification. McKinney PD was not able to make contact with
anyone at the residence. Collin County was notified to check the booking information of COLLINS for an
emergency contact. Collin County was able to retrieve information for his mother, SHARRON

unavailable to be reached via mobile phone (469-500-5183). Officers were able to make a next of kin
notification with COLLINS cousin, TAUSHA MOSTENBOCKER via mobile phone (972-679-4905). COLLINS
