Under Restraint: No Summary of Incident Summary of How the Death Occurred: (max. 30,000 characters) Summary: Kjeldgaard (here-in referred to as IM) intake on 1/5/18 no medical history noted other that documented above. Negative mental screening for magistrate notification. No record of sick calls or illness other than complaining of elbow pain diagnosed on sick call with synovial fluid on elbow 1/19/18. Cellmates report IM complaining of headache over weekend, but IM did not put in sick call. No abnormal behavior noted. Inmate accepted chow all day including meal at 1535hrs 1/22/18. DSO Rodriguez completed documented face to face check on 1/22/18 @ 1639hrs. DSO Rodriguez can be seen walking through E-Pod on surveillance up until 1649hrs. Camera in E-pod is PTZ camera and was facing away from cells towards crash gate so DSO Rodriguez' minute to minute location within the pod cannot be tracked. Cellmates advised IM was lying in bunk following chow. Inmates advised that at approximately 1715hrs, IM's arms became rigid (extended up and bent at elbow towards chest with hands clenched). IM began erratic breathing and then breathing would reportedly stop. One cellmate began shaking and trying to wake IM while others pushed call button for assistance. At approximately 1715hrs, the PTZ camera in E-pod spun towards the E5 cell call button as it is programmed to do when the call button is depressed. Cellmates alerted master control of IM in distress and unresponsive. DSO Rodriguez entered the cell at approximately 1717hrs to clear the cell for medical personnel and to provide assistance. On duty nurse arrived at approximately 1720hrs and began medical assistance (see attached nurses notes). ETMC EMS ambulance was dispatched, arrived (1735hrs) and transported IM to ETMC in Athens at 1741hrs. Detention Service Officers followed and stayed with IM (as practicably possible while medical treatment was occurring). The above times are attributed to the time stamps on the surveillance videos recording during the incident. Texas Ranger Michael Adcock, HCSO Investigator and Henderson County DA Investigator notified due to possibility of in custody death. Family of IM was notified and arrived at ETMC Athens. According to medical notes (attached), IM was transported to Medical City in Plano at 0815hrs 1/23/18 diagnosed with a ruptured aneurysm. DSOs were sent to hospital in Plano and provided direct supervision (again as practical based on medical treatments). 1/30/18 1320hrs IM was removed from life support devices and kept comfortable with pain relief/morphine following conversations between medical staff and family present. IM became deceased at 1725hrs same date. HCSO DSO Nicholas Garcia was officer present at actual time of death. At our request, the Collin County Medical Examiner's Office agreed to take possession of decedent and perform autopsy. ME's preliminary report has been received and listed death as Custodial Death Report :: Page 6 of 7 --- PAGE BREAK --- natural. Cause of death was listed as described above. Final HCSO and DA's reports awaiting final autopsy and medical records subpoenaed. Video recorded files preserved are too large to e-mail through our server and will be forwarded to via mail. Files, logs, reports and videos other than those attached will be provided upon request and will be retained and available for review to TCJS or O.A.G. personnel.
