Under Restraint: No Summary of Incident Summary of How the Death Occurred: (max. 30,000 characters) Summary: 2602-0254 - Info Report / Death Investigation 02/15/2026 1) I, Investigator C. Thom, was contacted by BCSO CIU Capt. Pate at approximately 1753 hours in regards to a death investigation. Capt. Pate informed me that there was a BCSO jail inmate who had suffered from a series of terminal medical issues and had just passed away in the HCA Clear Lake Hospital ICU Rm #318, located at 500 W. Medical Center Blvd. in Webster. TX. Capt. Pate advised that I will be met there by a Texas Ranger, which is common protocol when in-custody deaths were being investigated. 2) Upon arrival I entered the ICU Unit and as I reached room 318 I was met by BCSO Detention Deputy McDaniel #2264, who advised that he had been assigned to guarding the now deceased inmate, identified as Arturo Ramirez (DOB; 07/25/1957). Deputy McDaniel provided me with a paper with hand written notes that described everything that the medical staff had done to medically assist and/or render aid to Arturo Ramirez during the course of his tour of duty, which stated at approximately 0530 hours. 3) I then briefed with Deputy McDaniel and the RNs working the ICU wing. I was advised by Deputy McDaniel that Arturo Ramirez was admitted to HCA the day prior (02/15/2026) and that after some time in an emergency room bed he was transferred to the ICU. Deputy McDaniel then told me that Arturo Ramirez`s known medical problems include dialysis, diabetes and that he currently had 5 stents placed inside him. I was then advised by Deputy McDaniel that Arturo Ramirez while in the jail complained of chest pains, shortness of breath and high blood pressure, at which time he was transported to the hospital after being assessed by the BCSO jail infirmary staff. Deputy McDaniel than advised that the reported time of death according to the UCI RNs was 1727 hours. Custodial Death Report :: Page 6 of 8 --- PAGE BREAK --- 4) I asked Deputy McDaniel and the RN nurse (identified as Marco Huerta) if the next of kin had been notified. Marco Huerta informed me that the family had been notified and that Arturo Ramirez`s family had already been here. Deputy McDaniel provided me with the family member`s names who came to the hospital as Justin and Jeremy Ramirez, as well as Justin`s wife and son. Deputy McDaniel did not however get the date of birth or the phone numbers for the attending family members. I asked Deputy McDaniel if anyone had been in Arturo Ramirez room since he had passed away, to which he said no. Deputy McDaniel informed me that once Arturo Ramirez was pronounced deceased the room was cleared and nobody had entered it since. Deputy McDaniel did advise me that when the family was there they came to a consensus to take his father off of the ventilators, which was by being utilized to breathe for Arturo Ramirez. Deputy McDaniel told me that family gave a DNR for their father Arturo Ramirez. 5) I then pair of medical gloves and entered the room, where upon entering found an older Hispanic male subject with a gray hair and a gray beard laying on the hospital bed with a tube in his mouth. Based on my observation I found no signs of life on the male subject. As I analyzed further I found that the deceased male was missing his left leg from the knee down and that his right foot was missing two toes. I also observed an IV connected to decedents left wrist but I did not observe any fluids being given to him at that time. In addition the decedent had a breathing tube attached to his mouth and a medical tube running into his right nostril. The decedents left forearm, wrist and hand was swollen and slightly discolored, where I observed a medical wristband attached to his wrist. I was able to confirm the identity of the decedent as Arturo Ramirez by way of the wristband and matching him to the photograph in his jail booking photo. During my assessment of Arturo Ramirez and the hospital room I found no signs of abuse, neglect, suspicious activity or foul play. 6) I then took evidentiary photographs of the scene and Arturo Ramirez`s body, which I later uploaded into the case file. I then exited the room and asked to speak with the attending Doctor on the floor. I was then approached by Dr. Pumeet Singh, who advised that he was assigned to the floor. I asked Dr. Pumeet if he was going to sign off on the death certificate, which he stated that the Hospitalist Doctor would be. During which time Texas Ranger A. Arizmendi arrived, where he was briefed and he began with his investigation as well. 7) While speaking with the RN nurses Marco Huerta and later Cambrie Naquin, I learned that Arturo Ramirez had missed his dialysis treatment the day prior because he was unable to due to his current condition. I also learned that shortly after being admitted in the ICU Arturo Ramirez coded, where CPR was used to resuscitate him. I asked Deputy McDaniel if Arturo Ramirez had ever regained consciousness after being resuscitated, which he advised no. I was informed that after a pulse was re-established is when Arturo Ramirez was placed on the ventilator as a form of life support. 8) I later learned from the nursing staff that Arturo Ramirez was initially admitted to UTMB Angleton but was quickly transported via ambulance to this location being that were properly equipped to render the medical attention Arturo Ramirez required. I was then made aware that during the medical transport to HCA Clear Lake the EMS staff had to intubate Arturo Ramirez due to his condition. I then obtained a copy of the HCA death report completed by Marco Huerta and Cambie Naquin. The report showed that Arturo Ramirez was admitted for septic shock, abscess, chest and n-stemi but was told by Cambie Naquin that Arturo Ramirez had a number of contributing factors such as renal disease, hypertension, diabetes and dialysis treatment. I then asked Cambie Naquin for the name of the Hospitalist Doctor who would be signing off on the death certificate, to with I was given the name Dr. Raka Amin had agreed to sign off. 9) Based on my assessment of the scene, the condition of Arturo Ramirez`s body and the long list of life threatening medical issues Arturo Ramirez suffered from I found that no further investigation was required and in agreement with the medical staff that Arturo Ramirez passed away from natural causes. Since a doctor would be signing off on the death certificate no inquest by a J.P. was required, nor was an autopsy requested. Then at 1942 hours, Ranger Arizmendi and I released Arturo Ramirez`s body to the hospital where he would be placed in the morgue until the family could make the final arrangements. Cambie Naquin advised that she would be contacting the family to advise them that Arturo Ramirez`s was ready to be picked up by a funeral home of their choosing. 02/16/2026 10) I collected all the document and forms available to me and scanned them into the case file. In addition, I went to the BCSO jail infirmary and collected all the medical records the nurses had on file for Arturo Ramirez and uploaded them into the case file.
